Pharmacy Quality Scheme 2025/26 – Patient Safety Domain
The points allocation for meeting the Patient Safety Domain ranges for most pharmacies is 20.
Each point will be worth between £57.50 and £115.00 and so a contractor can earn between £1150.00 and £2300.00
Antimicrobial Stewardship
The aim of this criterion is to further support vital work to limit antimicrobial resistance (AMR) through effective stewardship within the Pharmacy First Service.
Contractors must complete a clinical audit of the clinical advice and consultations provided to patients scoring FeverPAIN 0-3 within the Pharmacy First Sore Throat clinical pathway. The audit must be carried out with a minimum of 10 patients over four weeks, or over an eight-week period if 10 patients are not achieved. It should be conducted from 1st September 2025 and completed no later than 31st March 2026 (this means the start date can be no later than 3rd February 2026).
A record of the start and end date of the audit is needed as this information will be entered into MYS during the declaration. The pharmacy must have completed the audit, sharing their anonymised data with NHS England, and incorporating any learnings from the audit into future practice by the end of 31st March 2026.

- Choose a suitable eight-week period to start the audit at some point between 1st September 2025 and 3rd February 2026.
- Make a record of the start and end dates of the audit for the MYS declaration.
- Reflect on the learnings from the audit and incorporate these into future practice.
Sepsis - CPPE Learning & E-Assessment
All registered pharmacy professionals working at the pharmacy on the day of the declaration must have satisfactorily completed, within the last two years (between 1st April 2024 and end of 31st March 2026), the CPPE sepsis online training and passed the e-assessment.
Actions
- Eligible staff to complete the CPPE training module on Sepsis and pass the e-assessment and show, as evidence, the certificate of completion.
- Use the Training Log to record numbers of staff who have successfully completed and passed the course.

Regularising Enhanced DBS Checks for Registered Pharmacy Professionals
All registered pharmacy professionals working at the pharmacy on the day of the declaration must have undertaken an enhanced DBS check to support the safe provision of clinical services, with a certificate issued within the last three years (between 1st April 2023 and end of 31st March 2026), to regularise the frequency of performing these checks in line with other healthcare professionals in the NHS.
Evidence is needed to demonstrate that all registered pharmacy professionals working at the pharmacy on the day of the declaration must have requested an enhanced DBS check, with a certificate issued within the last three years. This evidence must be available for inspection from the end of 31st March 2026 at premises level and must be retained for 3 years for PPV purposes.
